Nomination Form for Drawing Ration – 15 No Form Fill Up Process :-

1) First you need to download the form from the official website of the ration card or you can collect the form from the nearest food office or government camp at the door.
2) Then select the Aadhaar card that you could not link with your ration card for any problem.
3) Then Part 1A - Mention the names, ration card numbers, categories of all the consumers in the family.
4) Then put your contact information and address.

5) Then, besides you, who will be able to collect your ration, who will be the nominee, mention the name, ration card number, type, Aadhaar card number.

6) Then the applicant and the nominee will sign the form.
6) Then the form along with the documents should be submitted to the nearest food department office or door to the government camp.

But remember that you have to fill in the complete form in English.
